From Missed Refills to Restored Care: How ActualMeds Helped Close a Critical Gap

A missed medication refill can look like a simple adherence issue. But sometimes, it is the first sign that a member and their family are struggling with something much bigger.

That was true for one ActualMeds member whose wife had become his full-time caregiver as his Alzheimer’s disease progressed. When she called to refill his atorvastatin and lisinopril, our team found that both prescriptions had no refills remaining—and that his previous provider was no longer in practice. The medications supporting his heart health and blood pressure had quietly fallen through the cracks.

When a Refill Request Reveals a Bigger Need

As our pharmacist followed up, a more complex picture emerged. The member had been resisting appointments, while his wife was trying to manage his care, medications, and increasing resistance largely on her own.

The pharmacist escalated the case to the ActualMeds nurse care manager team, which focused on caregiver support and helped identify new provider options. Working with the health plan and its strategic partners, the nurse care manager helped coordinate access to telehealth and in-home provider care.

The Result: Support for the Member and the Caregiver

Within a week, the member was seen by an in-home provider. Both medications were refilled as 90-day supplies, giving the family much-needed breathing room. Just as important, additional case management support was put in place for the member and his wife, so she no longer had to navigate his care alone.

Why This Work Matters

Stories like this often do not show up in a claims report. They show up in a phone call that runs a little longer than expected, or in a pharmacist who asks one more question and uncovers the support a family really needs.

At ActualMeds, medication adherence is not treated as a checklist. It is an opportunity to identify risk, close care gaps, and connect members and caregivers with meaningful support before small issues become larger problems.

For health plans, this kind of follow-through can make all the difference for members and the families who support them. If your plan is looking for a partner who combines clinical expertise with compassionate, coordinated outreach, and care, we would love to talk.
